Performance Appraisal Forms
Appraisal Form: A hard copy of the Performance Appraisal form is available for reference. It is recommended that supervisors complete employee appraisals using the online Saba system.
Summary of Accomplishments (SOA) Form: A hard copy of the SOA is available for reference. It is recommended that employees complete their SOA using the online Saba system. However, the form is also available for those who are unable to complete their SOA using the Saba eAppraisal tool due to extenuating circumstances. Circumstances under which to substitute the eAppraisal tool include: An employee on an extended leave of absence, or an employee that has insufficient computer skills to complete the form. Once completed, the supervisor must upload the SOA into the documents section of the respective employees’ Saba account.
Probationary Assessment Form: In accordance with UC Merced’s performance management guidelines, probationary employees should receive a mid-probationary assessment. Once the probationary assessment is completed, the supervisor will need to upload the Probationary Assessment to the documents section of their respective employees’ Saba account.
