Injury & Illness Reporting Procedure

Report a Workers' Compensation Claim

 Supervisor 


 

1. Call 911 if a serious injury or fatality.

2. Have employee fill out the following:   

3. If the employee is not able to do so, "Create a Claim" by completing an Employee RSS Injury & Illness Report. Login with campus sign-on credentials and select Report Employee Injury or Illness from Quick Link Menu. 

4. Upload the incident report and DWC-1 to the RSS platform.

5. Complete Employer Investigation

6. Email benefits@ucmerced.edu if you have any questions 

* NOTE: Do not fax or email an Incident Report to Sedgwick or call the claim in. You can email benefits@ucmerced.edu if you have any questions.

   

 Employee 


 

1. Call 911 if a serious injury or fatality

2.  Immediately report the injury to your supervisor 

3. "Create a Claim" by completing an Employee RSS Injury & Illness Report. Login with campus sign-on credentials and select "Report Employee Injury or Illness" from Quick Link Menu.

4. Advise supervisor you have completed a RSS incident report so they can complete the Employer Investigation.

 

 

 

 

 

 

                                               

 

                                                                

 

 Office Manager, Admin, etc. 


 

1. Call 911 if a serious injury or fatality.

2. Notify employee's supervisor that an employee has been injured and the injury was reported. 

3. If the employee is not able to do so,"create a claim" by completing an Employee RSS Injury & Illness Report. Login with campus sign-on credentials and select Report Employee Injury or Illness from Quick Link Menu.

4. Advise supervisor you have completed a claim in the RSS System. so they can complete the employer investigation.

* NOTE: The Employers First Report must be completed within 24 hours of the University's first notice of incident. 

IMPORTANT 


Immediately contact Enviornmental Health & Safety (EH&S) if any of the following occurs; 

  • Worker fatality 

  • Inpatient hospitalization 

  • Loss of any body part (e.g., fingertip)

  • Serious injury

* These incidents must be reported to OSHA within 8 hours of the event. 

 

 

                                                                                                     

         

                                                          

 


Further Information 

Sedgwick CMS (Claims Management Services) administers the University's workers' compensation claims. A Sedgwick CMS claims administrator will investigate the injury and determine acceptance of the claim. If a representative from Sedgwick CMS contacts you, please provide any information requested. This includes departmental files, job description, or time records. 

Reporting an injury does not imply automatic approval of the claim. Claims are approved by Sedgwick CMS for UCM.
