EMPLOYEES

If you have experienced a work-related injury, and need medical treatment, please contact your supervisor for authorization.

If you wish to see your personal physician, you must have previously completed the Designation of Personal Physician for Worker's Compensation form and have it on file. Please inform the treating physician and other appropriate medical facility staff that the injury occurred while working.

For serious injuries not threatening to life and limb. Urgent care is available on a walk-in basis at the following facilities: 

* Please bring a copy of your completed DWC-1 to any of these facilities to receive care.
