Leave of Absence Process

 

Requesting a Leave Packet

If an employee needs to take three or more days off for medical purposes, the supervisor or employee should immediately contact the appropriate office. If the employee is aware of the need for leave in advance, they should contact the leaves office 30-60 days before the date that they need to go on leave. The leaves office will determine the employees' eligibility and send them a leave packet within 5 days. 

If the leaves office determines the employee is not eligible for FMLA, they will provide information about any alternative options available. 

During any FMLA leave, the use of sick and vacation time must be coordinated with the Leaves Office.

Going off Pay Status 

Coordinate with UC Path Center at 855-982-7284 to arrange to make your payments to continue your benefits when you are on a leave without pay. This includes the time that you are receiving disability payments from Lincoln Financial. The UC will pay the employer contribution for health care coverage while on FML, but you must pay the employee portion. 

You can choose to disenroll in benefits if you prefer at this time. You must contact UC Path within 31 days of your return to work to reenroll.

Changes on Leave

If your leave is extended beyond the date your health care provider originally identified, immediately notify your assigned Benefits Analyst and your supervisor.

If your leave was due to Pregnancy Disability Leave, or Parental Bonding, notify leaves office if any changes need to be made to the leave schedule due to the delivery date. 

Remember to add your newborn to your benefits within 31 days from the date of birth. This can be done on UC Path by going to Employee Actions > Health and Welfare > Life Events/Benefits Change. See the Having a Baby Roadmap on UC Net

Workers Compensation Leaves

Payroll procedures during a leave resulting from an injury while at work vary slightly from other medical leaves. While on a Workers Compensation leave, the employee continues to accrue full vacation and sick leave proportionate to the appointment percentage, and is paid for all holidays while on leave. The employee may use sick leave and vacation leave to supplement the Workers Compensation payments. All payroll procedures during Workers Compensation leave will be coordinated by the leaves office in conjunction with Payroll.
