- Sick Leave/Vacation Leave Usage
Determine schedule for time off and coordinate with supervisor - Complete Leave of Absence Request
- Arrange for Insurance Continuation and/or on-line payments of premium
- Provide Assistance with Disability Leave
Complete FMLA calculations - Complete Leave of Absence Request
- Arrange for Insurance Continuation and/or on-line payments of premium
